PREQUALIFY WITH THE BERG GROUP

SUBCONTRACTOR & SUPPLIER REQUIREMENTS

After submitting the form, our team will follow up via email with the next steps. As part of the pre-qualification process, you will be asked to provide the following documents:

  • W9 Form 
  • Audited or Reviewed Financial Statements (Previous Year)
  • OSHA 300/300A Logs (Past 3 Years)
  • EMR Letter (Past 3 Years)
  • Letter from Bonding Company
  • Current Insurance Certification and Additional Insured Coverages, either:
    • a) CG 20 10 11 85
    • b) OR CG 20 10 10 01 and 20 37 10 01, combined
    • c) OR equivalents in coverage in every respect as to option A or B
  • Diversity Business Certificate, if applicable (i.e. MBE/WBE/VBE certificate)
